If you are adding or changing more than basic information, you can
fill out a complete
registration form instead, only be sure to check the UPDATE box
in the top right section of the form.

This
may take a couple of minutes to download, your patience is appreciated.
2. Fill out all parts of this form with all known facts.
3.
Always include the child/adoptee's date of birth, country, and
state or province of birth on updates.
4. DO NOT abbreviate!
5. Fill out a separate
update for each child/adoptee or previous
separate registration you have on file at ISRR.
6. Please DO NOT send documents with this form
unless they have been requested by us.
7. Complete this form on-line, print it, then sign and mail it to the address shown above.
OR print it, fill it out using BLACK INK,
sign and mail it to the address shown above. Signatures are
required.

To receive a confirmation that this has been received you must enclose
a self-addressed, stamped envelope WITH the update. Otherwise,
ISRR will notify you only when a
"match" is made. Please do not
send mail that requires a signature or for our volunteers to wait in
line at the post office. It will not be received.
